Town carried a 1-0 lead into the second leg, thanks to Darren Bent's second-half winner at Portman Road.
The home side started brightly, with Baffour Gyan going close to levelling the aggregate scores in the 11th minute by shooting just wide of Andy Marshall's goal.
On the half hour Gyan headed over from Petr Papousek's corner, but Ipswich were more than holding their own. However, at the other end they had done little to worry home keeper Antonin Kinsky.
Seven minutes into the second half Gyan was again wasteful, heading wide of target from close range after a cross from Petr Johana, to let Ipswich off the hook.
Joe Royle then introduced Bent and Matt Holland on the hour to freshen things up, before Papousek drove another effort over Marshall's crossbar.
Fourteen minutes from time John McGreal headed away to safety as the Czechs again came close to levelling the aggregate scores.
Just as it looked as though Town would hold on, Gyan rescued Slovan with a headed goal two minutes from time to force an extra 30 minutes.
The first period of extra time yielded no more goals, but both sides created chances to take the aggregate lead.
A second 15-minute spell also failed to produce a winning goal, with a penalty shoot-out needed to decide the outcome.
From the spot, Jermaine Wright and Finidi George failed to convert as the Czechs won out 4-2 on penalties.
